The ancient mask is an item of heavy armour that is found towards the end of the DLC quest 'Qualian's last smile'. It is found in a crate alongside a corpse and a crafting blueprint for Qualian's Mask, a unique piece of heavy armour, which uses the ancient mask as one of its ingredients.
It looks like a dragon priest's mask from vanilla Skyrim, has a weight of 7 and a value of 150 pennies.
